      <title>Harm Reduction Outreach Worker - Casual (External)</title>

MAP aims to mitigate the health and social harms associated with AUD through alcohol prescription and scheduled delivery in the context of ongoing assessment and medical oversight, with attention to the social determinants of health.</p><p>The Map Support Worker will work with an interdisciplinary team to support community members enrolled in MAP who are experiencing social and physical harms associated with chronic alcohol use disorder and homelessness. Our team provides daily outreach services seven days a week, 365 days a year to MAP participants including individuals who are marginally housed and street involved. This service involves meeting clients where they are located, including in their homes, in housing support programs, on the street, in hotels and drop-in spaces.       <description><![CDATA[<p><b>Job Summary:</b></p><p>Brunswick Street Supportive Housing project provides 24/7, wrap-around support to 38 tenants who are 50+ to age in place. A multidisciplinary team of social workers, nurses, occupational therapist, physical therapist and continuing care assistants ensure each tenant has a support plan that meets the physical health, mental health and social needs. In addition, the team has housing support workers 24/7 to support tenants to maintain their tenancy and access services</p><p>The Physiotherapist (PT) is part of the Brunswick Street and NECHC collaborative primary health teams. The PT will work collaboratively with a team of physicians, registered nurses, nurse practitioners, social workers, dieticians, nurses, occupational therapists and non-health professional staff, they will also work alongside an additional 0.5 FTE clinic physiotherapist.</p><p>They will offer individual and if warranted, group physiotherapy to individuals who access one or more of the following services at the NECHC: NECHC primary clinic, MOSH, Brunswick Street, and Overlook. In addition to collaboration with staff and volunteers at the NECHC, collaboration with community agencies and systems (health, justice, and income assistance) will be required. The peer support worker will provide daily support, programming/groups, and mentoring to participants in various housing and community locations. This program is funded both by the federal government under the Homelessness Partnering Strategy and the provincial government. All activities associated with Housing First will be managed by the manager of Housing First.</p><p>The Case Manager will provide ongoing support to individuals to exit homelessness as they secure and establish permanent housing. Working within the Case Management model, they will support these individuals in creating a personalized plan and goals.
